St. Joseph’s Health has announced the launch of St. Joseph’s Health Partners, a clinically integrated network.
St. Joseph’s Health Partners provides coordinated care using data across providers and locations to deliver cost-effective and enhanced value health care to patients.
The CIN comes after a yearlong effort working with IPA of North Jersey, which represents more than 400 independent physicians affiliated with St. Joseph’s Health, engaging physicians from North Jersey to determine benefits for the community.
“This region is home to some of the nation’s best health care providers and, combining that experience with the breadth and depth of our collective resources, we will enable the participating groups to efficiently gather, share and utilize clinical data to identify and adopt patient-focused, evidence-based best practices,” St. Joseph Health Partners Executive Director James Giordano said in a release.
Targeted initiatives highlighted by the system include the management of chronic disease, preventative screenings, avoiding unplanned hospital readmissions, reducing costs and the overall improvement of health care quality.
