Horizon Blue Cross Blue Shield of New Jersey has signed an agreement with Aledade calling for 20,000 of commercially insured customers and members of Horizon’s Medicare Advantage to be included in Aledade’s New Jersey Accountable Care Organization, a value-based care model.
For Adelade, a Maryland-based company that offers value-based primary care models, the pact represents its first contract in New Jersey. Under its New Jersey ACO model, members will get to keep their primary care physicians, while Aledade will be responsible for the focus on care coordination, population health management and preventative care, as well as maintaining cost efficiency under its patient-centered delivery model.
"Our new agreement with Horizon is an exciting opportunity to advance value-based care and to improve the health of thousands of New Jersey patients,” said Dan Bowles, vice president of provider networks at Aledade. “In collaboration with local primary care practices, our innovative physician-led model will drive improvements in quality and patient outcomes through more personalized, coordinated care.”
